Child Support Services – Carver County
What We Do
Carver County Child Support Services helps with:
- Finding parents and establishing parentage.
- Creating and enforcing court orders for child, medical, and childcare support.
- Reviewing and modifying existing court orders.
- Working with other states to enforce support orders when one parent lives outside Minnesota.

How to Apply for Child Support Services
If you're not receiving public assistance, you need to apply for child support services.
You can get an application:
- From the Carver County Income Support Department.
- Print out the Full Child Support Services Application (PDF) from the DHS site.
If you use a paper application you can return it by mail or in person at this address 602 E. Fourth St., Chaska, MN 55318.
- If you are applying for children who have different fathers or mothers, complete an application for each parent who lives away from the home.
- Note: If you’re applying for children with different non-custodial parents, submit a separate application for each.
- There is no application fee.

If you're receiving any of the following public assistance programs, your case will automatically be referred for child support services:
- Minnesota Family Investment Program (MFIP)
- Medical Assistance (MA)
- Diversionary Work Program (DWP)
- Tribal TANF
- Foster care
- Childcare assistance

Request to Review Child Support Case - Parents may use this form to request that the county child support office review their court order to determine if it should be changed (modified). Parties can also respond to a request for review using this form. Financial Statement (PDF)

Child support ezDocs - The interactive Child Support ezDocs web tool helps Minnesota child support participants request or respond to a review of their current case. It also shows how a participant can file their own motion with the court to review their case.
