Every five years, Carver County Public Health conducts a Community Health Assessment (CHA) and develops a Community Health Improvement Plan (CHIP). This process helps identify local public health priorities and ensures that resources are focused on the greatest community and organizational needs.

The CHA helps us better understand the overall health of our community and pinpoint key areas for improvement. Insights from the CHA guide the development of the CHIP, which outlines specific strategies to improve community health. This work is done in collaboration with residents, community organizations, businesses, healthcare providers, faith-based groups, and policymakers.

One of the ways we learn about health in Carver County is by asking residents directly through a community survey. In fall 2023, a random group of residents received a paper survey in the mail. It included questions about physical and mental health, lifestyle, diet, and access to health care. The survey was completely anonymous, and the answers were combined to help us see overall trends in the community.

Survey responses provide valuable insights directly from residents about their health needs and concerns they observe in the broader community. These findings are a key part of the Community Health Assessment and help shape our ongoing public health services and priorities.
