Community Health Assessment

Every five years, Carver County Public Health completes a Community Health Assessment (CHA) to help understand the overall health of Carver County residents and the factors that affect their health. This includes information such as birth and death data, infectious disease rates, and chronic disease statistics. It also includes data on the conditions that shape health, like poverty, housing, income, race and other important factors. 

One way we learn about health in Carver County is by asking residents to share their experiences through a community survey. In fall 2023, a randomly selected group of residents received a paper survey in the mail. The survey asked questions about physical and mental health, daily habits, diet, and access to health care. It was completely anonymous, and all responses were combined to show overall trends in the community. 

The survey results give us important information about residents' health needs and the concerns they see in the community. These findings are a major part of the CHA and help guide our public health services and priorities.

Community Health Improvement Plan 

Information collected through the CHA is used to develop the Community Health Improvement Plan (CHIP). The CHIP is created through a community-led process and outlines steps to improve the health of Carver County residents.
