Birth Records
Carver County can issue birth certificates for people born anywhere in Minnesota. Current photo identification, such as a government issued photo ID, is required to obtain a birth certificate.
You can obtain a birth certificate in any county in the State of Minnesota for a person born in Minnesota after 1934. If you are looking for a certificate for birth prior to 1935, you must contact the county of birth.
Birth Certificate Request - Certified Copy
In Person - Carver County Land Records & Vitals Department- 600 East 4th Street, Chaska, MN 55318
- Complete a Birth Certificate Application Form (PDF) indicating that you are an authorized requestor. This form is also available in our office. Do not sign the form until you are with a staff member.
- Show a valid, government issued photo identification.
- Pay the corresponding fees.
- No appointment is needed. Must be present in office before 4:00 pm to guarantee same day issuance.
By Mail - Carver County Land Records & Vitals - Attention: Vital Statistics, 600 East 4th Street, Chaska, MN 55318
- Complete a Birth Certificate Application Form (PDF) indicating that you are an authorized requestor.
- Show a valid, government issued photo identification and sign the completed form in front of a notary public. Have the notary public sign the application and apply their notary seal.
- Mail the completed and notarized application along with the corresponding fees.
Confidential Birth Records
Some records are confidential and can only be obtained by some people.
Pursuant to https://www.revisor.mn.gov/statutes/cite/144.225, data pertaining to the birth of a child, to a woman who was not married to the child's father when the child was conceived, nor when the child was born, including the original certificate of birth and the certified copy, are confidential data unless the mother has designated the data as public.
Birth Certificate Request - Non-Certified Copy
A non-certified copy is a plain paper copy of a birth record. Non-certified copies are for informational use only and cannot by used for legal purposes.
