Carver County has access to death records of any person who died anywhere in Minnesota back to 1997 or any person who died in Carver County back to 1870. For death records outside of Carver County before 1997, please contact the County Registrar where the death occurred.

The Minnesota Historical Society maintains a death record index for most historical deaths. You can search the index online at the Minnesota Historical Society.

Death Certificate Request - Certified Copy

In person - Carver County Land Records & Vitals Department - 600 East 4th Street, Chaska, MN 55318

  • Complete a Death Certificate Application (PDF) indicating that you are an authorized requestor. This form is also available in our office. Do not sign the form until you are with a staff member.
  • Show a valid, government issued photo identification.
  • Pay the corresponding fees.
  • No appointment is needed. Must be present in office before 4:00 pm to guarantee same day issuance.

By Mail - Carver County Land Records & Vitals Department - Attention: Vital Statistics, 600 East 4th Street, Chaska, MN 55318

  • Complete a Death Certificate Application (PDF) indicating that you are an authorized requestor.
  • Show valid, government issued photo identification and sign the completed form in front of a notary public. Have the notary public sign the application and apply their notary seal.
  • Mail the completed and notarized application along with the corresponding fees.
