  • How do I record my ordination certificate?

    Bring in your ordination certificate/documents and a valid government issued ID/driver's license.  This must be done at least one day prior to performing a ceremony.  If registering by mail, mail in a photo copy of your ordination certificate/documents and your driver's license. The cost is $20.00.
  • How do I get authentication of a birth or death certificate or Apostilles?

    Apostilles or other authentications of birth or death certificates to be used for official purposes in other countries are obtained through the Minnesota Secretary of State. Check with each country's consulate or embassy to determine that country's requirements for an authentication to be valid.  Information about obtaining an apostille or other authentication of a birth or death certificate is available from the Minnesota Secretary of State's website.
  • How do I register my Notary Commission?

    To register in person, go to the county in which you reside.  Bring the letter/card issued from the state; a valid government issued ID/driver's license and a check, cash or money order in the amount of $20.00. 

    To register by mail, send in the letter/card issued by the state, a photocopy of your valid government issued ID/driver's license, a check or money order in the amount of $20.00 and your signature notarized by another notary on a signature card or letter.
  • How do I get a certified copy of a death certificate?

    Deaths which occurred 1997 or later are available statewide.  For deaths that occurred prior to 1997, you need to go to the county in which the death occurred or the deceased's county of residence at the time of death.  You will need to provide a valid government issued ID/driver's license and complete a form proving you have tangible interest as described in MS 144.225 Subdivision 7.  The cost is $13.00 for one and $6.00 for each additional copy of the same record. 
  • What do I do if something is misspelled on my birth certificate?

    If the birth occurred prior to 2001, go to a registrar office to see if it would be a correction or an amendment.

    If the birth occurred 2001 or later, an amendment would need to be done.  You can go to the nearest local registrar office which issues birth certificates and ask for an Amendment Packet.

    The amendment process is completed through MN Department of Health. 

  • How do I get a birth certificate if I was born in another state?

    You will need to contact the state where you were born.
  • How do I get a copy of my birth certificate?

    If you were born 1935 or later, the birth certificates are available statewide.  You will need to provide a valid government issued ID/driver's license and complete a form proving you have tangible interest as described in MS 144.225 Subdivision 7.  The cost is $26.00.

    If you were born before 1935, contact the county where you were born.

  • How do I get a certified copy of my marriage certificate?

    Copies can only be issued from the county where you applied for your marriage license.  You will need to fill out a request form. The cost is $9.00 for each certificate. 
  • What are the hours of operation?

    Office hours are Monday – Friday, 8:00 am – 4:30 pm, except for holidays.  Please arrive no later than 4:00 pm to allow time for processing your request prior to closing.
